This form may only be used to request a withdrawal from all classes. To make changes to specific classes, please contact the front desk.
Permanent withdrawals last longer than two months, and a registration fee must be paid upon return.
Temporary withdrawals last between two weeks and two months.
Withdrawals must be requested at least two weeks before the first day of the month in which the change is to take effect, to avoid being charged in-full for the next month (e.g. plans to drop a class late-April need to be addressed by mid-March, in advance of posting April's tuition).
You may not file for a withdrawal in the current month of classes.
Makeup slips cannot be used while a student is withdrawn.
